Incident Narrative
An employee had swapped out the punch and die on an ironworker and was aligning the punch to the die. As the employee was bump cycling the machine, the edge of the punch struck the edge of the die and the punch broke. A piece of steel from the punch struck and punctured the employee's right eye.
