Incident Narrative

An employee had been dumping trash into a compactor. While returning to the building, he tripped over a bent piece of plate metal that serves as decking for a platform. He fell to the concrete floor and sustained fractures to the left wrist and right knee, and also sustained a cut above the left eye.
