Incident Narrative

Employees were making emergency repairs to the girder of a bridge that was struck by a tractor-trailer. The injured employee was waiting for grinding to be completed so he could stick-weld. He was standing next to a hanging torch when a spark from the grinding caused leaking propane to ignite. The employee sustained a flash burn to his right ankle and required hospitalization.
