Incident Narrative
At 11:45 AM, on August 19, 2022, two employees arrived on-site to install a brake hub and brakes on a pump jack using a cherry picker. While performing work on the pump jack, the arms of the pump began moving and struck the bucket of the cherry picker causing an employee to fall from the bucket and sustain a broken neck.
