Incident Narrative

An employee was readying a machine to operate when he noticed the trim suction of the right tube was not as strong as the suction of the left tube. Finding a small gap where the tube was seated in the chopper inlet, he began to re-insert the tube back into the inlet. He then noticed a piece of tape on the edge of the inlet; as he was removing it, the suction from the trim blower pulled in his left hand. The trim chopper blade partially amputated his index and middle fingertips.
