Incident Narrative
A 16,000-pound hammer crew set up a tool on top of another tool to remove the scales/trash from the bottom tool. The hammer began to operate. On the second punch, the top tool flew off and punctured an employee's upper left leg, breaking the thigh. The employee was hospitalized.
