Incident Narrative
An employee was using a step ladder to retrieve a box from the top shelf in a kitchen storage area. As the employee was coming down the ladder, she put the box she retrieved onto a lower shelf so she could step off the ladder without something in her hands. As she stepped back, she missed the last rung of the ladder and fell backward onto her left side, resulting in a dislocated left ankle that was also broken in three spots.
