Incident Narrative

An employee was processing merchandise and needed more totes to continue his task. To retrieve the totes, the employee used the conveyor belt to access the dock below to where the totes were located. His foot became caught between the links of the conveyor belt, which caused lacerations and muscle damage to his lower leg.
