Incident Narrative

On August 4, 2022, at approximately 8:00 AM, a steel erection crew was installing decking on a roof that was approximately 40 feet high. An employee stepped on an unsecured piece of sheet metal and fell to the concrete surface on the ground level. The employee sustained fractures to their nose, upper jaw, and both legs. The employee also hit their head and sustained back injuries.
