Incident Narrative
An employee was working from a scissor lift at a height of about 6 or 7 feet. He became dizzy, lost consciousness, and fell forward off the lift. The railing briefly stopped his fall, but he fell head-first onto the floor (a fall of about 4 to 5 feet).
