Incident Narrative

An employee was using a portable ladder to position and tack-weld steel panels onto sections of a ship. A panel was suspended from an overhead crane, and the employee was positioning it. As he was ratcheting the panel, it pinched his thumb against the ladder. The tip of the thumb was amputated above the nail.
