Incident Narrative
On July 15, 2022, a ready-mix truck driver was at a job site pouring concrete. He attached an extension chute to the primary chute (needing to force it into place because of concrete buildup) and began lowering the chutes. His right hand was caught between the two chutes, and the index fingertip was amputated.
