Incident Narrative
An employee was performing a video safety audit of a telecom tower. He lost balance while transitioning from the second to the third step of a ladder. He reached out with his right hand to regain balance and stepped off the ladder. His right little finger got caught between two angles of the telecom structure and was amputated at the knuckle closest to the palm.
