Incident Narrative

An employee was using an air wand to clear debris around equipment. An unguarded belt and pulley caught the sleeve of his safety vest and pulled his left arm into the conveyor end. The employee was hospitalized with a lower arm fracture and bilateral arm lacerations requiring surgery.
