Incident Narrative

An employee and co-worker were working from an operator's basket of a rough terrain forklift to install plywood sheathing on the exterior of the second floor of a townhouse. The employee fell out of the basket approximately 10 feet to the ground and then tumbled down the sloped side of the ground about another 9 feet, resulting in a broken ankle that required hospitalization and surgery. Fall protection was not worn at the time of the incident.
