Incident Narrative

While performing an hourly operational check, an employee noticed a blower was not running. The employee went into the equipment room, opened the motor control center (MCC) and reset the circuit breaker inside the MCC to restart the blower. An arc flash occurred, causing burns to the left side of his face and ear, second degree burns to his left hand, and third degree burns to his left arm.
