Incident Narrative

An employee was standing on the side of a flatbed trailer, trying to pull a tarp over the load on the trailer. The tarp was stuck, and as he pulled on it he slipped and fell off the trailer to the ground. He suffered broken ribs, a dislocated right shoulder and elbow, and internal bleeding in the head.
