Incident Narrative

The injured employee was helping to build a large differential (drive train for a vehicle) on a table. He was trying to fit the gears into the housing, then went to adjust a shim (washer) and asked a coworker to make an adjustment with a 3-pronged bar tool. The injured employee's right middle fingertip became caught inside the differential and was amputated.
