Incident Narrative
A temporary employee used a hoist to load a roll of product onto a machine. The employee then lowered the roll to the ground to reposition it. After repositioning, the employee activated the hoist using the remote in her left hand. The chain of the hoist tightened around her right index finger and amputated the fingertip to the first knuckle.
