Incident Narrative

On May 25, 2022, at approximately 9:00 a.m. employee 1, a boom truck helper, was working with a boom truck operator. They were unloading a pallet of shingles onto the roof of a single-story, new construction home. The employees were utilizing a crane mounted to a flatbed truck. Employee 1 was near the ridge cap of the roof unloading the pallet of shingles when their foot slipped and the employee slid down the roof on their back and landed on their feet, approximately 10 feet below. The employee suffered a fractured T12 vertebra, a fractured left heel, and a spinal cord injury from waist down. Neither employee was utilizing fall protection at the time.
