Incident Narrative

An employee was clearing jammed parts from the work deck of a powdered metal press. The rotary wheel that moves the parts into the finished part bin had tension in it and rotated, pinching the employee's finger between the edge of the wheel and the frame of the press, resulting in an amputation. The machine was not guarded at the time of the incident.
