Incident Narrative
On May 9, 2022, an employee set up a 24-foot fiberglass extension ladder three rungs above the edge of the roof and set the ladder's footers parallel to the brick driveway. The employee was ascending the ladder to inspect the electrical circuit inside the soffit prior to repair when the ladder slipped and he fell about 3 feet to the ground. The employee suffered a broken left ankle.
