Incident Narrative

An employee was on a 6-foot ladder that was standing on a piece of plywood, which had been placed on joists at the second-floor level of a building. The piece of plywood moved out from under the ladder. The employee fell off the ladder, through the second-floor joists and then through the first-floor joists, and onto the concrete basement floor 14 feet below. He suffered a head contusion.
