Incident Narrative

An employee was trying to install a glass door. The door was in the open position and would not close. When it was forced shut the stainless steel lamination peeled off its bottom rail. The employee was trying to remove the lamination when it cut his fingers. He suffered a partial amputation to the right little finger (down to the first knuckle), as well as a laceration to the right ring finger.
