Incident Narrative

Two employees were working in an area. The first was using a pneumatic press to consolidate explosive powder into a pressed pellet. The second was using a manual arbor press to extract the pressed pellet from the pressing die. There was an explosion of zirconium potassium perchlorate, and the second employee suffered burns to the face and neck.
