Incident Narrative
At 11:33 a.m. on April 20, 2022, an employee was using a forklift to load freight onto a trailer, a small 26-foot box with a roll-up door. The door had fallen slightly when the employee was trying to get into the trailer, and he had pushed it up. While he was in the trailer on the forklift, the door lowered again. As the forklift was backing out of the trailer, the employee reached up from the forklift to push up on the door. The forklift moved backward at the same time, rising as it moved onto the ramp. The employee's hand was crushed between the metal frame at the top of the forklift and the metal frame of the trailer.
