Incident Narrative

In the feedwater basement, an auxiliary pump had been put into place for temporary drainage into an approximately 20-inch-deep trench. The grate guarding the trench was askew to allow the hose to feed under the grate and into the trench. An employee tripped on the 1.5-inch discharge hose from the auxiliary pump. This caused the grating to dislodge, allowing the employee's foot to submerge into 180 to 190 F wastewater in the trench. The employee sustained second-degree burns to the lower left leg.
