Incident Narrative

An employee was working in a loading area on a 10-foot ladder to help lower an empty 4-foot by 4-foot agricultural bin that had become lodged between a forklift's forks and a dump truck. The bin was lowered to the ground and the forklift began to back away from the truck. This caused the employee to lose his balance and fall, landing on the sidewalk 8 to 10 feet below. He suffered a 1-inch laceration to the forehead, a stress fracture to a small bone in the wrist, and a chip in the bone connecting his hip to his pelvis. He was hospitalized.
