Incident Narrative
An employee was cleaning spilled glue from a concrete floor. He mopped a thin layer of water over small areas of the hardened glue to help thin it. A few minutes later, he slipped on the floor and fell. He landed on a tape measure clipped to his right hip and suffered a femoral neck fracture that required surgery.
