Incident Narrative

An employee was conducting maintenance on a jammed garage bay door. The employee was trying to dislodge the door by pulling on a rope when the door suddenly loosened and came down with enough force to send it back up its tracks. The top part of the door disconnected from the railing, swung back, and struck the employee. The employee suffered injuries to two neck vertebrae.
