Incident Narrative

An employee was working on a steel plate, about 1 foot by 8 feet, to be installed on a structure that was being fabricated. He stepped on a piece of metal that had been welded to the plate, causing the plate to strike him in the shin. He suffered a laceration and was hospitalized, requiring surgery.
