Incident Narrative

An employee was working from the boom basket of an aerial lift. As he lowered the lift, the boom landed on a metal standpipe. The water hose and nozzle at the standpipe's top rim broke under the weight of the boom; the boom shifted and dropped the basket to the ground 12-18 inches below. The employee suffered a broken left femur.
