Incident Narrative

An employee saw fumes coming off a filter canister and tightened the bolts on top of it. When he turned to walk away, the bolt sheared and he was sprayed with titanium tetrachloride (TiCl4). He suffered chemical burns to the left wrist and forearm and was hospitalized.
