Incident Narrative

Two employees were loading a roll of plastic film into a machine. The machine cycled while one of them was dislodging stuck film from it; its forming bucket moved upward and pinched the employee's right hand against the forming head severely injuring the fingers and resulting in an amputation. The machine was guarded at the time.
