Incident Narrative

An employee was performing break-in maintenance in the hub of a turbine when the blade suddenly pitched back. The employee reached for a hydraulic tool in an effort to prevent the turbine from being damaged. The employee's left ring finger got caught between the tool and the blade's stationary locking pin position housing, resulting in amputation of the left ring finger.
