Incident Narrative
An employee was pulling a pallet of product onto a lift gate using a manual pallet jack. He turned it to the left so he could reach the controls to operate the gate. Once the back wheels came off the trailer and onto the gate, the momentum picked up and pushed him backward. He tried to use the left side chain for support, but the pin came out of the top bracket and the chain came loose. He then fell to the ground about 4 feet below, injuring his right hip and wrist.
