Incident Narrative

An employee was preparing to close a foldable, approximately 16-inch wide dock plate mounted to the lift gate of a box truck. He took a step forward with his right foot and his right leg fell through a 3- to 4-inch gap between the warehouse dock and the lift gate, resulting in an impact injury to his abdominal area.
