On February 21, 2022, an employee was standing on the forks of a pallet jack to retrieve an empty pallet from a chest-high stack of pallets. The employee stepped backward, tripped over the fork, and fell. The empty pallet landed on the employee's right leg, causing an injury that required hospitalization and surgery.
