Incident Narrative
An employee was attempting to release a pivoting fifth wheel plate on a frameless dump trailer. The pin would not release, so he asked another employee to put the truck in reverse to relieve pressure on the pin while he used a hammer to gain leverage on the releasing mechanism. The pivoting plate caught his right index finger and amputated it below the second knuckle.
