Incident Narrative

An employee was climbing a ladder to access a single-story roof to investigate a leak. As the employee approached the top of the 12-foot climb, the ladder base kicked outward and the employee fell to the ground, landing on his feet. The employee was hospitalized with a broken right tibia.
