Incident Narrative
The injured employee was using a crowbar to remove shims from beneath a 9,000-pound plate. The crowbar kicked out causing the plate to shift back on the shims. The employee's hand was caught between an aluminum shim and the table and their little fingertip was amputated below the nailbed.
