Incident Narrative
Employee 1 was running product out of a machine in order to clear the machine for cleaning while Employee 2 was grabbing the product out of the machine. The main fan was not engaged while the employee was running material through the machine. Therefore, the machine began shooting fiber into the air and visibility became very poor. Employee 2's right hand contacted the rotating mechanical part (smash roller) and his right index and middle fingers were amputated. The smash roller spins at 1500-1800 RPMs.
