Incident Narrative
An employee was redressing a 3.5" setting tool. In order to stroke the tool, he slammed the end of the tool down on an outrigger mat. His right ring finger slipped inside the setting sleeve on top of the tool. The mandrel inside moved upward when the tool was slammed and amputated his right ring fingertip.
