Incident Narrative
A Turner employee had been assigned to remove exterior piping, valves, and flanges on a tank. The employee noticed flames coming from erosion holes in the top of the tank and evacuated the work area, headed for a maintenance shop. As they reached the shop, the tank exploded. The employee was knocked down by the blast and burned by the flames.
