Incident Narrative
An employee was attempting to make repairs to a warehouse heater. The employee utilized a 15-foot telescoping hydraulic lift that was positioned under the junction box. The employee boarded the lift and raised the platform to a height of 9 feet. He then reached back to secure the swing gate of the platform when it jostled the linchpin securing the enclosure to the work platform. The release of the linchpin caused the enclosure to collapse, knocking the employee off the elevated platform to the concrete floor. The employee suffered fractures to both wrists, their right knee, and their right ankle, requiring hospitalization.
