Incident Narrative
An employee was on a landing above a frac tank. The employee put a strap stick into the tank; when it hit the bottom of the tank, the fluids in the tank ignited. The flash fire caused the employee to jump or fall, and he suffered two broken legs as well as burns to the arm.
