Incident Narrative

An employee was packaging birdseed when a piece of poly got stuck inside the machine. The employee attempted to clear the piece off the machine when a pulley on the machine contacted their right index finger, resulting in a fingertip amputation. The machine was not locked out/tagged out at the time of the incident.
