Incident Narrative

An employee was preparing to re-assemble a conveyor extension. She lifted the conveyor extension to place the arms in the bracket and lost control of the extension, causing the weight of the conveyor extension to shift and pinch her right ring finger against the conveyor frame. The employee sustained a distal phalanx fracture and her right ring fingertip was amputated.
