Incident Narrative
A Charles E. Jarrell employee was on a 10-foot A-frame ladder, removing a hanger for refrigeration lines that had been hung from a concrete ceiling. Some of the fireproofing material that had been sprayed onto the ceiling and the refrigeration lines fell onto the employee's face, causing him to fall off the ladder and suffer a broken tibia.
