Incident Narrative
On December 15, 2021, an employee was assisting in a process change evaluation. Two cylindrical adapters were to be joined together with a 40lbs. spacer in between. Several bolts, which were used to secure the components together, had damaged threads. In the process of removing the bolts, the spacer came down. The adapter slid down and pinched the employee's left index fingertip between the flange of the adapter and the metal plate, resulting in an amputation.
